Financial / Parenting Disclosure Statement
When Parenting/Financial Disclosure Statements are Required
- Child Support Determinations
In cases involving joint children where a child support order must be determined, Minnesota Statute § 518A.28 requires each party to serve and file a financial affidavit with their initial pleadings or motion documents. This affidavit must disclose all sources of gross income and include supporting documentation necessary to calculate parental income for child support purposes
- Temporary Financial Relief Motions
When a party requests temporary financial relief—such as child support, spousal maintenance, payment of debt, or attorney’s fees—Rule 303.02(b) mandates that the Parenting/Financial Disclosure Statement be served and filed by both the moving and responding parties, along with their motions and affidavits .
- Pretrial Conference Requirement
According to Rule 305.01 of the Minnesota General Rules of Practice, each party is required to complete a Parenting/Financial Disclosure Statement in the form developed by the state court administrator. This statement must be served upon all parties and filed with the court at least seven days prior to the date of the pretrial conference
Reality of Parenting / Financial Disclosure Statement
The document was created with pro-se parties in mind as a document they could complete that would provide the court will all the information they would need to address any financial issues. Realistically, we don’t often complete these documents because we have or will separately provide info to the court (paystubs, budget, taxes, debt asset sheet, etc) that more accurately and thoroughly answers the questions outlined in the parenting and financial disclosure statement. While we can’t/won’t say you don’t need to complete this as stated above, it is not uncommon for this document to not be completed.

Completing the Parenting / Financial Disclosure Statement
See MyCase for a template of the document.
The template/document is pretty self-explanatory. If you are completing this document AND have submitted exhibits to the Court, it is generally okay to reference (and in some cases attach) those exhibits in lieu of filing out sections. Examples would include:
#5 Monthly Living Expenses – reference and attach budget
#6 Monthly Withholdings – reference and attach paystubs
#7 Real Property – reference debt asset sheet, mortgage statements, purchase documents
#8 Personal Property (a-j) – reference and attach DA sheet
#9 Non Marital Claim – reference exhibits for non-marital, expert reports if applicable
#10 Life Insurance – reference life insurance statements, face sheets, DA Sheet
#11 Debt – reference and attach DA sheet
#12 Documentary Information: Provide your three (3) most recent paystubs from your employment, your most recent Federal Tax Returns with all attachments, including W-2s and 1099s, and any statements from unemployment compensation, workers’ compensation, social security benefits statements, and all other documents evidencing earnings or income received during the last three months, including any public financial assistance in money or in-kind services (grants, heating assistance, rental assistance, etc.) – we should have these already for trial and can attach these.
Filing the Parenting / Financial Disclosure Statement
File the document as you would any other document. Eserve on OPA or, if OPP is pro-se, then mail to them with an affidavit of mailing. Note that many of the attachments you attach will be confidential, so you’ll need to complete the cover sheet and file some of the docs confidentially. See separate wiki article on how to do that
