Confidential Information Form 11.1 (Social Security Numbers)
CONFIDENTIAL INFORMATION FORM 11.1 is the form that is where we put the parties and children’s Social Security numbers. Gen. Prac. Rule 11 covers this.

This must be done by the Petitioner in every new family case
- CHILD: we only put minor children on this form, not children who are 18 and have graduated highschool.
- SSN: put the parties and children’s Social Security Numbers. If the person is not a u.s citizen and does not have a SSN, then put “none”.
- NOTE: if we don’t know the SSN, put “Unknown”. We try to avoid this by asking our client if they know social security numbers as part of the intake, but if they don’t know, then we can put “unknown”
When filing this document with the court, it is one of the few documents that will be filed as Confidential
